Assessment Coordinator (RN/Licensed Therapist)

The Assessment Coordinator is responsible for assisting in the process of responding to referrals and other requests for services and information. The Access Counselor provides technical assistance to the discharge planning process, under the direction of the Director of Assessment Referral. This position is also responsible for completing applicable portion of Core Measures (HBIPS) Abstraction Tool for all admissions. This is a remote position. Candidate must have a TX license.

Work hours: Monday – Friday (11 a.m. – 7:30 p.m.)

Job duties include:

• Documenting and processing inquiry calls in accordance with intake policies.
• Assessing and stabilizing patients prior to leaving hospital or arranging hospital admission.
• Notifying on-call doctor and assisting with patient interviews.
• Obtaining administrative approval for all admissions.
• Completing admission paperwork in accordance with hospital policy.
• Completing intake log with all information documented.
• Making follow-up calls to those failing to show up for appointments.
• Obtaining pre-certification in accordance with insurance requirements.
• Completing outcome studies forms.
• Demonstrating guest relations and counseling employees in need of training.
• Notifying administration of problems or significant events.
• Identifying safety problems and taking corrective actions.
• Obtaining medication after hours as ordered by the physician with proper documentation.
• Completing Core Measures Data (HBIPS) Abstraction Tool for all admissions.

Other duties as assigned or directed by Director of Intake (Market).

Qualifications requirements include:

• Education: Registered Nurse or Master's Degree in a human relations or other related field.
• Experience: Two (2) or more years of experience in intake assessment activities within an acute psychiatric setting. A minimum of two (2) years' experience in a psychiatric hospital setting including customer contact.
• Licensure: Licensed to practice in the State of Texas as a mental health professional. (RN, LMSW, LCSW, LPC or LPCI)
• Additional requirements: A working knowledge of the DSM-IV and the Mental Status Exam. A working knowledge of the Addiction and Recovery field. May be required to work flexible schedule and overtime as needed; may be required to work on-call as needed.
